Educational case of endoscopic sinus surgery indicated for a suspected right maxillary fungal ball. Preoperative CT, selected surgical clips at original speed, later CT, and specimen analysis are shown.
The operative report describes uncinectomy, wide maxillary antrostomy and anterior ethmoidectomy, with purulent material and solid debris. Pathology described chronic sinusitis with mild acute activity; no fungi were identified on PAS-diastase or Grocott stains. Culture grew Serratia marcescens. The preoperative suspicion of a fungal ball is therefore not presented as a confirmed diagnosis.
At the first follow-up, the clinical record documented a good postoperative course, an open antrostomy and no endoscopic signs of infection.
