English on-screen text.
Educational case of a 54-year-old man with fever, facial pain, purulent rhinorrhoea and forehead swelling. CT describes left frontal sinusitis, osteomyelitis with anterior- and posterior-table erosion, and a 16 × 40 mm subperiosteal collection. No clear intracranial extension is identified in the supplied report.
The video combines preoperative imaging with selected endoscopic surgical excerpts. The operative report describes bilateral endoscopic sinus surgery and septoplasty, right Draf IIa and left Draf IIb frontal sinusotomy, endonasal drainage, drilling and a steroid-eluting implant. The excerpts do not document the complete technique or every step; details taken from the operative report are identified as reported information.
The intraoperative culture grew Cutibacterium acnes, considered possible contamination. Histopathology describes chronic sinonasal inflammation with acute activity. Reported follow-up describes good clinical progress, patent cavities and a clean frontal sinus. Follow-up is presented as text: no follow-up images were supplied.
